AI systems are increasingly being proposed for clinical decision support. However, their integration into patient care raises important ethical questions. This work provides concrete frameworks for ensuring AI in medical diagnosis serves patient interests and respects human values.
We identify five core principles that should guide AI development in healthcare: beneficence (maximizing benefits), non-maleficence (minimizing harms), autonomy (respecting patient choice), justice (fair access and benefit distribution), and transparency (interpretable decision-making).
We provide concrete implementation guidelines addressing model selection, validation, integration, and monitoring. Key recommendations include: requiring clinician approval for all AI recommendations, maintaining detailed audit logs, and establishing independent oversight committees.
Importantly, we argue that AI should augment rather than replace clinical judgment. Even high-accuracy models should be positioned as decision support tools with clear limitations and uncertainty quantification.
Effective ethical governance requires clear accountability structures. Institutional review boards should assess AI systems similarly to clinical trials. Clear liability frameworks should be established for AI-related adverse outcomes, with responsibility shared between developers, deployers, and clinicians based on appropriate factors.
